Are you noticing visible veins on your legs that make you feel self-conscious? You’re not alone. Many adults experience vein problems that affect both appearance and comfort. Understanding whether you have spider veins or varicose veins is the first step toward finding the right treatment.

Understanding the Basics: What Causes Vein Problems?

Both varicose veins and spider veins are caused by valvular failure within the vein. Your veins contain one-way valves that help blood flow in one direction, back to your heart and lungs. When these valves break down, gravity pulls blood backward and holds it in the damaged vein.

The main cause of vein problems is genetic – it usually runs in the family. However, female hormones, pregnancies, and direct trauma to a valve can also play a role. Once the problem starts, it will most likely slowly get worse under the influence of gravity.

What Are Spider Veins?

Spider veins are the smaller, more superficial veins that appear directly on your skin’s surface. Here’s what you need to know about them:

Appearance and Characteristics

Spider veins are often very small and appear blue, red, or purple in color. They look like thin spider webs or tree branches spreading across your skin. Sometimes they are even mistaken for a large bruise.

Symptoms

While spider veins are less likely to cause symptoms because they are smaller, they can still feel tender like a bruise. If they get extensive enough, they can cause restlessness and cramping when you are trying to go to sleep at night.

What Are Varicose Veins?

Varicose veins are larger than spider veins and often cause more noticeable and severe symptoms.

Appearance and Characteristics

Varicose veins refer to the larger, bulging, rope-like veins that form due to breakdown of the Saphenous leg veins and their branches.

Symptoms You May Experience

Because varicose veins are larger, they contain more blood, and this results in a higher chance that people with varicose veins will start to have symptoms. Common symptoms include:

  • Aching and pain
  • Heaviness
  • Fatigue
  • Itching
  • Burning sensation
  • Cramping
  • Restlessness
  • Swelling
  • Throbbing

Because it is a gravity dependent disease, symptoms are typically best in the morning and worse late in the day, correlating with how long the leg has been down.

Potential Complications

If left untreated some patients will eventually develop areas of bleeding, blood clots, or even open ulcers near their ankles.

Key Differences: Spider Veins vs. Varicose Veins

The main differences between these two conditions are quite distinct. Spider veins are much smaller and appear as thin, web-like patterns on the surface of your skin in blue, red, or purple colors. They typically cause minimal symptoms and rarely lead to complications. In contrast, varicose veins measure over 1mm in diameter and appear as bulging, rope-like structures that are usually blue or dark in color. These larger veins can be located beneath the skin or on the surface, and often cause significant symptoms like pain and swelling. They can lead to serious complications if left untreated.

What Causes These Vein Problems?

Primary Risk Factors

The three most important causes for varicose and spider veins are: family history of vein disease (80% or more); female gender (20%); and rarely direct trauma to a valve (<1%).

Contributing Lifestyle Factors

Several lifestyle factors can increase your risk:

Age: The aging process causes a general decline of all the bodily systems over time, and such is true of the veins as well.

Inactivity: If you do not have an exercise routine or move your body regularly every day, it may place you at greater risk to develop varicose or spider veins.

Occupation: If you work in a field that requires that you spend most of your workday on your feet and standing in one place, this may be a factor that plays a part in the development of your veins.

Obesity: Being overweight does not cause your veins to break, but it can cause things to get worse faster. Extra weight places more pressure on the valves, and if you are predisposed to varicose veins, or if you already have them, it will cause things to get worse more quickly.

Treatment Options Available

Spider Vein Treatments

Sclerotherapy: Sclerotherapy is the most useful procedure for treating spider veins in the legs. In a sclerotherapy treatment, a sclerosant is injected into the veins to scar them down and cease blood flow.

Surface Laser Treatment: Surface laser and broadband light treatment is used to treat the smallest spider veins, which are typically red or purple in color. Surface Laser Treatments tend to work best for facial veins.

Varicose Vein Treatments

Endovenous Thermal Ablation: Endovenous thermal ablation can be performed using either laser fiber or a radiofrequency catheter. It is used to treat the large Saphenous trunks historically reserved for surgical stripping. During a thermal ablation procedure, a fiber or catheter is inserted directly into the vein, local anesthesia is placed around the outside of the vein, and laser or radiofrequency energy is used to permanently seal the vein shut.

Ultrasound-Guided Foam Sclerotherapy: Foam sclerotherapy is used to treat the remaining varicose veins branches after thermal ablation takes care of the larger trunks. Since the FDA approved medication for sclerotherapy is a detergent, it can be foamed into a shaving cream consistency. When injected into a vein this foam damages the lining of the vein wall, causing it to scar down and eventually go away. The foam is highly visible with ultrasound, so it allows physicians to be much more accurate and comprehensive in their treatments than ever before.

Ambulatory Microphlebectomy: In a microphlebectomy procedure, small incisions are made in the skin just next to the vein under local anesthesia, and the vein is then removed through these incisions. This is most useful for small sections of vein when a cosmetic quick fix is the goal.

When Should You Seek Treatment?

If you have varicose veins, spider veins, or highly visible veins on your face or hands, you may benefit from one of our vein treatments. For most patients it comes down to appearance and discomfort, and once they are fed up with one or both, they will seek treatment.

Why Choose Professional Treatment?

It is very important to treat your varicose veins because, if left untreated, they will only continue to look and feel worse. The good news is that today’s treatments are safer and more effective than ever before. Physicians like Dr. Erdman, who specialize in vein treatment, will typically provide more comprehensive care (treating for both symptom resolution and cosmetic appearance) than physicians who are hospital-based that are only treating with the goal of resolving symptoms.
